Convenient Electronic Forms
With just the simple click of a mouse, our secure online service allows you to fill out the required online forms before your child's scheduled appointment.
To help us get to know your child, please take a moment and fill out our electronic patient information and health history forms at least 24 hours before your appointment. If you have any questions along the way, feel free to contact our practice.
We kindly request this form be completed for all new patients and existing patients at each recall visit. You will receive a text or an e-mail prior to your appointment with a link to the electronic forms and with information about the virtual check-in process. You can also access the forms here:

If someone other than the parent/guardian is bringing the child to the appointment, please complete the form included that authorizes someone else to bring your child to the dentist.
